Intensive Outpatient Program and Case Management
We recognize that a veteran’s after-treatment plan can be equally as or even more important than the In-Residence treatment program. For this reason, we focus extensive resources and expertise in formulating a comprehensive, detailed continuing-care plan for every veteran to insure lasting healing. Depending on the unique requirements of each veteran, we create a rigorous, structured plan for her/him to follow, after graduation. Every transitional treatment plan is unique, but may include the following services:
Intensive Outpatient Treatment Services at Solara
Graduating veterans may continue to participate in daily programming at Solara, which is largely the day program segment of the Partial Hospitalization Program, including:
- Regular Psychiatrist Follow Up and Continuing Medication Management
- Continuing Individual Therapy with the Primary Therapist
- Daily Small Group Therapy
- Case Management
- Luncheon
- Adjunctive Therapies
- Program Activities
- Alumni Activities
Our Admissions team can provide the cost and addition information regarding our daily 3-hr. or 6-hr outpatient programs.
Continuing Case Management
Our expert case managers can provide case management services to our local veterans. Case management may include the following, predicated on the desired level of service:
- Daily Check-Ins
- Weekly Treatment Plan and Veteran Schedule Review
- Communication with Medical and Clinical Providers
- Housing Assistance
- Transportation Assistance
- Education Assistance
- Employment Assistance
For veterans who are not local to us, the Case Manager creates an extensive post-treatment plan with services in the veteran’s home area, which may include:
- Psychiatrist Referral
- Clinical Referral
- Transitional Housing Recommendation
- Outpatient Program Referral
- Support Group Referral
- Coaching and Monitoring Referral
- More
Professional Monitoring Services
Graduating veterans, either local or not, may elect to matriculate in a ‘veteran monitoring program,’ provided by independent monitoring groups. Depending on the veteran’s needs, these services can be exceedingly helpful in week-to-week, day-to-day, or even hour-to-hour veteran assistance.
Solara’s Intensive Outpatient Program and Case Management for Veterans
Solara Mental Health in San Diego is focused on giving veterans a supportive and safe environment for their recovery. Our mental health treatment facility recognizes that each veteran will have their own personal needs, and we will customize their treatment plan to help with their specific diagnosis and symptoms.
Solara Mental Health is VA-contracted, which gives us the unique opportunity to help veterans with their mental health needs. As long as authorization is approved, we provide housing, transportation, and accommodations.
Alumni Program
The alumni of our mental health treatment center are a wonderfully diverse and highly supportive group of graduates. They can be found locally, as well as throughout the country and beyond. All of our alumni share a common bond and lasting friendships develop regularly, whether or not they were in treatment at the same time. All can participate in the program, either through our robust local activities, or remotely through social media. We provide regular local services and activities for all. We also provide communication, education, interaction and more through social media and email channels. Our alumni program is an outstanding vehicle for veterans to remain connected to us and to each other in a supportive, fun way.
